An unexplained, box-like outline beneath President Trump's suit jacket has sparked renewed online speculation, after being captured on camera for at least the third time in recent months.

The latest sighting occurred on Wednesday as the 80-year-old president travelled to North Carolina to campaign for Republican Senate nominee Michael Whatley. Footage posted to Instagram by White House aide Margo Martin showed the distinct rectangular shape visible beneath the back of Trump's jacket as he spoke to reporters near Air Force One. The outline was first noticed during a 60 Minutes interview in April, and appeared again last month in footage from a rally in Long Island, shared by former White House Press Secretary Karoline Leavitt.
What the White House said
When The Daily Beast asked the White House to explain the shape, spokesman Davis Ingle did not identify what it was, instead firing back at the outlet directly. He said: "Only the glue-sniffing degenerates at the failing Daily Beast would obsessively push baseless conspiracy theories about President Trump, and that's because they suffer from a severe, crippling disease known as Trump Derangement Syndrome that has completely rotted their peanut-sized brains." He went on to contrast Trump's health with his predecessor, adding: "President Trump's sharpness, unmatched energy, relentless work ethic, and historic accessibility stand in stark contrast to what we saw during the last administration when Democrats and the legacy media intentionally covered up Joe Biden's serious mental and physical decline from the American people."
What's actually known about Trump's health
The White House has not identified what causes the visible shape. Separately, in July 2025, White House physician Captain Sean P. Barbabella disclosed that Trump has chronic venous insufficiency, a common and generally benign condition, after Trump reported mild swelling in his lower legs. Barbabella said testing at the time found no evidence of deep vein thrombosis, arterial disease, heart failure, renal impairment, or systemic illness. No public evidence has linked that disclosed condition to the jacket bulge specifically.
